Is it unsafe?

Before I use it, I'll rinse it out with hot water, and the smell is gone, but after I'm done, emptying it, and keeping top off, the next day it still smells musty.

Almost like wet clothes left in dryer.

There's never been anything in it other than straight water.

Is it unsafe?

What is it

It's probably a result of bacteria growing inside which is a common problem with standing water. If you're just spraying your plants with it there's no harm, but to get rid of the smell completely you'll have to sterilize the inside with something like bleach. I use a hand spray bottle myself and generally avoid this by being careful not to let any part of the inside (particularly the tube that draws water up) touch anything when I'm refilling it.
