Nepenthes are vining plants and can pretty much get as big as you let them over time
Pitcher size in the other hand is a different story, different species get differently sized pitchers
The largest pitchers of those mentioned are definitely from truncata

LFSM works perfectly fine. Some folks go 50/50 LFSM/perlite to make the media more airy. Live LFSM is also used quite a bit for top dressing.
