I'd say that is a Nepenthes miranda

I agree: Nepenthes miranda is a hybrid between N. maxima x N. northiana.

And I would say it's N. 'Mixta' x maxima. I believe that the N. northiana influence is slightly more prevalent than if it was N. maxima x 'Mixta', but this topic is strongly debated.
