They open before the cilia are formed, and the part where the cilia are connected dies. Also, one side opens before the other. Both are doing this. There’s a lot of new growth, but they keep doing this. It isn’t root rot.

Looks more like mineral burn. Maybe try to flush the media.

Google images of "vft mineral burn" and you'll see a host of different examples, some of which look like what your trap is experiencing.

Not necessarily, it all depends on what your media was in the first place or what/where the minerals are coming from.

This may or may not solve the problem. It really just depends on the amount of TDS in your water.
First, try buying distilled water. If you're currently using distilled water, buy a new bottle.
Next, order a TDS meter to measure the amount of nutrients in your soil/water.

If your water has a higher TDS than 50, the cause of this nutrient burn is your water. If you flushed your plant and the TDS is greater than 50, your soil is to blame.

If I am right, the plant is from SNW which means that it is potted in 50/50 peat/perlite. In that case, your media isn't the problem and is most likely your water source. You haven't indicated what water you are using, or if you have tested the TDS of it. With that size pot, I think your best bet is buying a gallon of distilled water and flushing a half gallon through each pot over the period of a day. You want the water to grab ahold of any minerals that are already there and drain them out the bottom of the pot. That being said, when you flush it you don't want the pot sitting in a tray where the water will accumulate. Once you have the soil flushed, you need to address your water source and either get it tested, or switch to bottled distilled water or rainwater.
