Been having this guy for a few years now. I would put it outside when the weather was warm enough, and it thrived. Then, eventually the weather would get too cold and I would bring it inside by a partly sunny window. All seemed OK until I decided to repot it (as I'm full bare root repot into fresh media) around September. It never bounced back from the repot, and I honestly thought it was dead. Upon inspecting it today, there seems to be a little green left on it near the growth points. I'm putting it in "plant intensive care" under a growlight and partially bagging it for higher humidity. I'm keeping my fingers crossed, but I will be surprised if it pulls through. Last photo is what it used to look like.

Is there a possibility the new grow media somehow had fertilizer in it? what did you use? Maybe you damaged the roots when you repotted it?
Did you use a fresh pot / sterilized media ?

All good points, Supercazzola! I'm gonna say no way to the possibility of fertilizer in the media. However, I have never bothered to sterilize any media for my plants, sorry to say. Or the pots. I usually just kinda swab them out with rainwater and get the "crud" out of them. As far as the roots, I was as gentle as possible with them, but we all know how delicate and brittle Nepenthes roots can be. In hindsight, I wish I had just put the root ball with some of the old media into a bigger pot with fresh media. But, too late for that! The only reason I even repotted it was because the plant was getting large and lopsided and had been blown down by the wind a couple of times. Also, unfortunately there was a cool snap that came through the night I repotted the plant. Just a perfect storm of how to murder a nice plant, I suppose. Guilty as charged. Lol.

Update: Sadly, my "Raff" did not make it, despite my best efforts. I really wasn't too optimistic, but I had to try. Dug it up yesterday and it was D-E-D! That's the way it goes, sometimes, I guess. If anyone has a spare cutting, I will buy or trade generously for it! Thanks

Sorry to learn of it. I recently lost two new N. Hookeriana. I’m devastated, as it was the first time I lost a Nepenthes within a week of getting them from the seller.

Sorry to hear that. It sucks to lose a plant that soon. Any idea what happened? The plants may have had a disease or condition before you got them?

I have a Rafflesiana same type as yours that put up a basal. The plant's stem got etiolated from growing past the light so I'll probably be cutting it off very soon after I finish hardening it off. If you can't get one by then I'd be happy to send you part of it

OK, so massive thanks to Apollyon for sending me 2 HUGE vine cuttings of his N. Rafflesiana! Super guy! And he wouldn't even take a dime for the plant or shipping! Hoping I can get a strike from these. Will keep y'all posted. There are still good people in this world today!

Thanks! I actually do have an update. 2 of the 3 cuttings are toast. 3rd one (the one I showed the least attention and special treatment) is showing good signs of striking from 3 different points! Hoping for the best!
