My flytraps are dying?

They were doing fine until around late may, then they suddenly almost completely died. The leaves are very dry and floppy, its still growing new traps but they are extremely tiny and turn black before completely forming.

A few questions.
1. How long have you had it?
2. What kind of soil is it in?
3. How much sun does it get?
4. How often do you water it?
5. What kind of water?

Ive had them for a year now, its in a mix of peat moss and perlite, I keep it under two grow lights that are on for about 12 hours a day, I water it every couple of days when it starts to get less damp, and I only use distilled water.

Could be too near the light.

Sounds like the time I burnt my plant to death.

Any changes recently to the light intensity/ distance?

Looks like either they got cooked or they have mineral burn.

Do the ingredients in the soil have added nutrients?

Do all of the new traps start turning black? I just noticed that a new trap on mine started turning black but I'm thinking its heat from the sun.
