Just as an indicator, how long does it usually take for a healthy plant in near perfect conditions to form pitchers (from leaf formation to fully developed pitcher)?

the growth of the pitcher will totally depend on your conditions you provide the plant , if its close to there natural habitat climate , they can be very quick matter of weeks to a month , but if you have it all wrong it can take months ,and it all so as stated depends of the type of plant you have as well , a lot of hybrids from there pitchers much quicker than specie plants most of the time as there usually more vigorous growing plant ,with exceptions !!.
