To me it looks like the newest open leaf has already decided not to pitcher. The next leaf has potential if the conditions are ok. It looks like there was a recent change in conditions a few leaves back but the newest growth looks healthy so far.

Definitely not. If it was going to pitcher, the tip would be curling down and getting wider. That's a dead tendril.

It's really hard to tell sometimes, one of my nepenthes always seems to grow stunted pitchers, then they just rapidly start growing them at the last minute.

now that i've witnessed a few pitchers forming, i do see the nubs of a pitcher shape there. they are very cool. they start tiny and don't stop growing until they are surprisingly huge.
